Renewal Instructions

Online renewal is no longer available.

The Board Office will send out renewal forms approximately 60 days before the expiration of your license.

Initial licenses expire on September 30th of the year following the year of issue thereafter they expire every year on September 30th. Notices are emailed to each licensed Athletic Trainer at their last email address of record approximately sixty (60) days prior to September 30th. 

The renewal application must be post-marked by September 30th of the renewal year to be considered. Any renewal application received with a post-mark after September 30th is returned, and the license becomes void.

When the renewal application is approved, the license is renewed for one (1) calendar year.

The renewal period is closed. 

Late Renewal -  An expired license may be renewed within 90 days of its expiration.  

Reinstatement - Licenses not renewed within 90 days must apply for reinstatement. The reinstatement form is available on the Forms page.
